How to report fraud
There are four ways to report fraud to the Fraud Hotline:
- Toll-free phone number (1-888-82 FRAUD) available for anonymous phone calls
with access to investigators during business hours and voice mail after hours.
- Send an E-mail message to the Fraud Hotline: fraudhotline@gbbragg.com
- Fax the hotline at: (916) 783-0338.
- Fill in the form below:
Instructions
In order to gain a better understanding of the situation you are reporting,
please describe in detail the event(s) of fraud that you believe to have been
committed. Please include known information about the suspect, such as their
name; home address; telephone number; approximate age or date of birth; the
name and address of their employer(s); when the event(s) occurred; identity
of other individuals involved, i.e. co-worker, physician, etc.; the identity
of other witnesses; and what kind of benefits or moneys the suspect is believed
to be receiving. Please include any additional and vital information that
you think would be helpful to the investigation. If you would be willing to
later testify to the information that you have just given please include your
name, address and a current telephone number.
